This is a simplified Description of Benefits. This is not an Insurance Policy or Certificate. Benefits are administered according to the provisions of the Group Master Policy and premiums are charged according to the filed rates. The information contained in this explanation is representative only, and in all cases insureds should refer to their Certificate for coverage specifics. Some provisions, benefits, exclusions or limitations listed therein may vary depending on the insured's state of residence.
IMPORTANT NOTICE: THIS CERTIFICATE DOES NOT PROVIDE COVERAGE FOR SICKNESS OR ILLNESS.
If you were to die suddenly due to a covered accident or suffered a disabling injury, you or your beneficiary will receive the benefit amount you chose upon purchasing coverage. This lump-sum payment can then be used for immediate or future needs. Once enrolled, you will be covered 24 hours a day, 365 days a year from an accident that occurs on or off the job.
AD&D insurance benefits are paid in addition to any life insurance benefits and can help safeguard your family’s lifestyle if the income you provide is no longer available. AD&D coverage provides benefits if an accident occurs, but not if death occurs naturally. Unlike life insurance, AD&D insurance provides benefits if the insured loses a limb, sight, hearing, or speech from a covered accident.
Your status as a union member means you and your spouse can purchase valuable AD&D protection at affordable group rates. Group AD&D insurance typically offers lower rates than you could receive as an individual for the same coverage and benefit package. Group insurance also means you can never be singled out for a rate increase, or a change in benefits, even if your health changes as you grow older.
Your rates for coverage are based on the amount of coverage you apply for and whether you choose individual or family coverage. Rates can only be changed if changed for everyone under the Group Master Policy.
The premium is the payment you make for insurance coverage.
Your beneficiary (ies) is the person(s) you name in an insurance policy to receive the death benefit.
Coverage is available for members and their spouses ages 18-69.
Your coverage provides 10% of your AD&D benefit for unmarried, dependent children who are full time students. 1
Coverage continues as long as premiums are paid when due, you remain a union member, and the group plan remains in force and is not modified by the policyholder to end insurance for the group of insureds to which you belong. Coverage will expire for you or your spouse at age 80.
During a union-sanctioned strike or lockout lasting 30 days or more, your premium will be waived for up to 12 months one time within a consecutive 12-month period.*
